The minimum monthly program rate for our wellness program is $40.00 per week, billed in monthly increments.

Taxes will be applied upon completion of your mailing address.

We do not ship to P.O. Boxes and a signature is always required.

Once you have paid the minimum program fee, please email your first and last name, and order number.

A representative will contact you directly to go over the additional rates available and send you an invoice for the difference.

Once your balanced is paid in full, your product(s) will ship within 5-7 business days.
